PATIENTS & CAREGIVERS

Medicare covers your home health services for as long as you are eligible and your physician orders home health services. We work closely with your physician to ensure that you continue to receive the services you need. Our staff is licensed, highly skilled and fully trained, available to serve our patients seven days a week.

WHAT TO EXPECT

We work with your physician to provide the care you need from the comfort of your home. You will receive a complete order misoprostol online written plan of your care, which will explain all the services we will be providing to you.

You can expect the following:

• We will start your care within 24 hours of you being discharged from the hospital or receiving orders from you physician.

• We will review all your Medications.

• We will complete a Full Health Assessment.

• We will complete a Home Safety Assessment.

• We will answer any questions you have.

We have a dedicated Registered Nurse on call 24 hours a day to answer any questions you may have.  We will coordinate your care with your physician to ensure you are getting exactly what the doctor ordered.  Your physician can login into our Physician Portal 24 hours a day to view the progress of your care.
